4 Paws for Ability provides life-changing service dogs for children and veterans. All applicants, regardless of their race, color, national origin, sex, age, disability, or religion, are treated with respect and privacy. Applications are accepted solely based on our ability to meet the client’s needs. 

Your service dog application has three components: the application, followed by a health form, and reference letters. You may save your application at any time to complete later if needed, and a copy will be emailed to you.

Child Application
